Patient Account Terms
Terms governing patient portal and online booking account credentials managed by Scribeberry.

These Patient Account Terms (the "Terms") govern your creation and use of a patient portal login or other authentication credentials managed by Scribeberry Inc. ("Scribeberry", "we", "us", or "our") to access a clinic's patient portal or complete online booking that creates an account on the Scribeberry platform (your "Patient Account").

These Terms cover authentication and account use only. They are not consent to treatment, not a clinic's cancellation or fee policy, and not a substitute for intake forms or clinical consents your clinic may require.
By creating a Patient Account or using portal sign-in, you agree to these Terms. Your use of clinic services is also subject to your clinic's Patient Terms, Privacy Policy, and any booking terms or consents you accept with that clinic.
1. Relationship to your clinic and Scribeberry
Your clinic provides care and controls your clinical records. Scribeberry provides software and manages Patient Account authentication so you can securely access clinic-hosted portal features.
How Scribeberry processes authentication and platform data is described in the Platform Privacy Notice at https://scribeberryclinics.com/legal/privacy-notice. That Notice is incorporated by reference; key privacy details are not repeated in full here so a single privacy document can stay current.
Questions about appointments, clinical content, invoices, or clinic policies should be directed to your clinic.
2. Eligibility and account information
You must provide accurate contact information and keep your email address up to date so security and appointment messages can reach you.
If you create or manage an account for a minor or another person for whom you have legal authority, you represent that you have that authority and that your use complies with the clinic's policies and applicable law.
3. Account security
You are responsible for maintaining the confidentiality of your password and for activity under your Patient Account, except for activity caused by our failure to safeguard the Platform.
Use a strong unique password. Do not share your login. Notify your clinic and, for suspected platform credential compromise, privacy@scribeberry.com or security@scribeberry.com, if you believe your account was accessed without authorization.
We may require multi-factor authentication or additional verification to protect your account.
4. Acceptable use
You may use your Patient Account only for lawful personal (or authorized caregiver) purposes related to care at clinics you are connected to. You will not attempt to access another person's records without authority, disrupt the Platform, upload malware, or misuse messaging features.
5. Availability and changes
We aim to keep authentication and portal access available, but the Platform may be interrupted for maintenance, security, or factors beyond our control.
We may update these Terms by posting a revised version with a new "Last updated" date. Material changes may also be called out at sign-in or by email where appropriate. Continued use after the effective date constitutes acceptance.
6. Suspension and termination
We may suspend or terminate a Patient Account if we reasonably believe it is used unlawfully, presents a security risk, or violates these Terms. Clinics may also disable portal access according to their policies.
You may stop using your Patient Account at any time. Closing a Patient Account does not delete clinical records held by your clinic; ask your clinic about chart retention and access.
7. Cross-clinic access
Today, Patient Accounts are used in the context of the clinic portal or booking flow you are using. If Scribeberry later offers a single login across multiple clinics, we will describe that feature and any additional terms or consent at the time it is introduced. Until then, do not assume that one clinic can see another clinic's records through your login.
8. Disclaimer and liability
Scribeberry provides authentication and platform software. We do not provide medical or psychological care. Clinical services and related liability are between you and your clinic.
To the maximum extent permitted by law, Scribeberry is not liable for indirect or consequential damages arising from Patient Account use. For platform authentication services, our aggregate liability is limited to CAD $100 or the amount required by non-waivable law, whichever is greater. This limit does not affect rights you may have against your clinic, or liability that cannot be limited by law.
9. Governing law
These Terms are governed by the laws of the Province of Alberta and the federal laws of Canada applicable there, unless mandatory consumer protection laws in your province require otherwise for that subject.
